This is a database and form used to collect information by our Suicide Prevention Officers. These Officers roam our suicide dorm and had to hand write information about the inmates activity every 10 to 15 minutes on as many as 24 inmates. The paper work was never ending. I was given the task to find a better way to record this information. I turned to the IPAQ Pocket PC and Handbase forms for help. This form allows the officer to click on a few fields and move on to the next inmate. The date and time are automatically recorded when the form is started. We then store the database on our network for easy lookup. This form is totally unlocked. We password protect ours so no one can change the submitted times, dates, and activities.

This is a zip file that includes the database file, the form file, and power point presentation on why we moved to this format and a small instruction sheet. REVISED ON 04/04/03

This is a good starting point for using this in your case-tracking work. There is a database for keeping a listing of Judges, one for Attorneys, and also a linked action list to keep track of what was done, and what's left to do. Perhaps some attorneys, etc, will be able to improve this for their needs. If you do so, please email me at: dhaupert@ddhsoftware.com with your applet or submit a new one here on the Gallery!

A quick reference guide for law enforcement officers. Contains over 350 entries in less than 300kb! Almost every traffic law is listed. Contains the statute number, catergory (moving,non-moving,criminal,etc.), points applied to license, exact text of statute and notes section to add data. Database is catergorized for easy searching. Also contains forms for easy viewing. Has 19 seperate views already inputted based on category of violation. Statute text and notes are seperated onto seperate forms to not clutter the record view. Now includes a dictionary lexicon for legal definitions. The lexicon allows one to look up words from within Handbase and determine the specific definition according to FL Statute.

The primary database is "Police." It is used as a launching point to the "child" databases. The relationship used is "date." The relationsihp field type is used to enter/view information related to BOLOs, citations, contacts, field interviews, notes, service calls, subpoenas, time worked, to do, and vehicle unit. The "linked" field type is used to edit the "child" databases.
The "child" databses use "time created" as their primary relationship with the "PoliceNotes" "child" database.

This is a very easy to use database. Logging all contacts during which you administered the SFSTs can allow you to testify, if you ever need to, that you have conducted SFSTs, how many you have conducted (at least since you began logging), that you don't arrest everyone you test and that you (hopefully) have a pretty good record of arresting only people who ultimately prove to have BACs over the legal limit.
