Description:This is a database and form used to collect information by our Suicide Prevention Officers. These Officers roam our suicide dorm and had to hand write information about the inmates activity every 10 to 15 minutes on as many as 24 inmates. The paper work was never ending. I was given the task to find a better way to record this information. I turned to the IPAQ Pocket PC and Handbase forms for help. This form allows the officer to click on a few fields and move on to the next inmate. The date and time are automatically recorded when the form is started. We then store the database on our network for easy lookup. This form is totally unlocked. We password protect ours so no one can change the submitted times, dates, and activities.

Sorting:You can sort by Officer, by inmate master number, by cell number, by date, time, and inmate activity. We download the information to an Access database and use Infomaker to generate a cleaner report than what HanDBase offers.

Comments:This form was built using the demo hanDBase enterprise edition. I have since edited the form under the registered addition that we purchased. This is a zip file that includes the database file, the form file, and power point presentation on why we moved to this format and a small instruction sheet. REVISED ON 04/04/03
