Welcome to the worlds first online handheld runtime development center. This tool requires the original databases to have been developed using HanDBase.
The runtime application has been divided into seven sections: General, Trial/Full, Databases, Startup, Help Text, Icons, and Final Build.
Each section has its own instructions so you'll know exactly what to do before moving to the next tab. Please start at the General tab and work your way right to finish with the Final Build tab.
1. It is powered by HanDBase 4.0, the best multi-platform database in the world!
2. Other medical softwares only allow you to record your admitted patients only. Patients Files will allow you to record both your In and Out patients!
3. It will allow you to easily encode details through its customizable pop-up lists. Thus, typing the same words again and again will be avoided. Fit for busy doctors.
4. The design will make your PDA look classy!
5. The attractive image icons will allow you to easily navigate the different details of your patients.
6. It contains an IMAGE section where you can capture pictures of your patients under their database for easy reviewing. Through it, you can get a snapshot using your Treo Phone camera and it will automatically be logged in under the specified patient. Or you may install the pictures through a hotsync for other Palm devices that has no camera and will still be viewed under the corresponding patient.
7. The following data can be encoded: Name, Demographics, Allergies, Family History, Personal Medical History, Chief Complains, Diagnosis, Date Admitted and discharged, Date of Visit, Physical Exam, Laboratories, Medicines given, take home medicines, Progress Notes, Procedures, Images, Vaccines given and next vaccine, Professional Fees for both admissions and clinic visits.
8. It can be viewed and edited in the desktop using the Handbase 4.0 desktop companion. You may add data through the PDA and the Desktop and do a hotsync. The end result is that both platforms will have each others files joined together! Made possible by the two-way synchronization system!
9. Through the Desktop Companion, you may be able to export your files (ex. Admitted Patients details from May 1 to 31) to one tabular EXCEL file. Great for census presentation and filing!
10. It is designed to be applicable to both residents and consultants of any specialty and of any country!
11. Because it is powered by Handbase 4.0, you will be able to do the following:
a. I want to filter and view the details of all my presently admitted patients.
b. I want to recall what vaccines had I scheduled to be used on May 5.
c. I want to review the details of all the past admissions and visits of John Smith.
d. I want to know how many patients that I saw who had bronchopneumonia for the whole year.
e. I want to print the admitted record of John Smith.
f. I want to know the total fees from my admissions for the month of June.
g. I want to beam my patients’ data to another resident.
h. I want to know who are the patients who hasn’t paid me yet.
i. I want to know who were the patients that I scheduled for follow-up today.
j. All these and more...as you continue using it, you will realize all the other functions that will greatly help you! Understand particularly the FILTERING and the RUN REPORT functions and you will go a long way!

2003 CPT CODES ADDED. I have added the ICD 9 codes and V codes for OB/GYN with links to each one of the fields Surgery, Deliveries, Hospital Admits, and Consults. You can search for the ICD 9 codes and Cpt codes or use the old format I originally developed. OB ICD 9 codes may not have the 5th digit included but are followed by a + indicating another digit is necessary. The available 5th digits are included in the description eg (0-4) or (1,3) etc. Common language has been substituted in many of the ICD 9 descriptions and several codes not related to OB/GYN are included in the database. Use the FIND feature of the HanDBase program to facilitate quick location of codes, remember to check SEARCH ALL FIELDS when utilizing this feature. If you don't like the order of the fields, go to properties and click on edit field order. Suggestions welcome.
This ap is for the busy clinician who sees a number of patients a day and needs a convenient way to make sure necessary follow-ups are done with regard to lab tests, X-Rays, etc. Records include reason for follow-up, action taken, contact info, etc. Makes a good 'peripheral brain' for making sure all follow-ups are done (and a record that they were!). Version 2 corrected a problem with field lengths and added a 'delete' record button for each record. Version 3 removes the 'delete' button and gives the user the ability to save 'done' records to a seperate database entirely. This gives the user a more concise look at those follow-ups still not done. It also allows the 'done' database to be archived, printed, exported, etc. for future reference.

This applet was developed to allow fast, simple collection of birth data by CNMs in conjunction with the ACNM Benchmarking Project. My goal is to make it as easy as possible for many more CNMs to collect data about their practices and submit it to the ACNM Benchmarking project. This program allows data bout a birth to be entered into a handheld in a matter of minutes. Data can then be compiled on a weekly or monthly basis by each CNM, or can be synced into one databse for the practice.
Contains fields for date, primary & delivery CNM, maternal age, number of prenatal visits & US, delivery mode, gestational age, Apgars, birth weight, NICU admission, epidural use, perineal outcomes, LOS, and maternal & newborn readmits.

It has addressed several deficiencies of the previous version. It is still not the ideal logbook but atleast it is simple & easy to maintain. There is no point in collecting lots of useless data which is not useful at a later date. What a trainee needs to collect to demonstrate proficiency in a particular procedure is different from what a consultant needs to collect. This database is just a skeleton - it can be personalised fully.

A value of -1 indicates that no datum was available for that antibiotic / organism combonation, while a number from 0 - 100 is the per centage of the isolates that were sensative to the given antibiotic for that organism. Set a filter prior to doing a report. Since some puff fields may have a data value of zero; a missed reading, make sure you make a filter to exclude zero values because they will otherwise be included in the average calculation which will reduce your real puff average. I set the minimum value to something over 0. You can also set a filter to calculate through min and max dates. When this is coupled with the minimum value field filter, you can then generate a report for your readings since your last doctor's office visit that represents your true readings' average.

Written by a mom, not a doctor. Designed to help parents track the various environmental factors that may contribute to their child's negative behaviors, as well the interventions they tried, and the child's response to those. It is hoped that this database will help parents get a handle on what triggers their child's tantrums (or other negative behaviors), to better make sense of their child, and to figure out what actually helps.
Has Data:
Yes
Has Forms:

Filters:
Filter by Name (in case, like me, you have more than one child on the spectrum), by date (is he worse on Mondays? Is she worse on a monthly basis?), by time of day (could this pattern mean he needs a nap at this time?), by type of adverse behavior, by its severity and duration (Now I see that he's worst under these circumstances), by prior activity (oh, he most often tantrums if I take him away from the TV), by whether a transition is involved, and what it was, by how the child was approached before the behavior (Now I see that he screams if I approach him in a hurried manner), by various environmental factors (such as noise, lighting, clothing, the familiarity of people, locations, and tasks), by whether the child was tired or hungry, by interventions tried, and by the child's response to interventions (hmmm, I usually try hugging him, but he responds better to being given some space).
Comments:
There are lots of Pop-Ups and there is one sample record included. In addition to the factors mentioned above, there is a place for noting the child's self-comments, as these can be enlightening. There is also a place for making general notes. It is NOT the purpose of this database to tell parents how to deal with their children. It is rather a venue for allowing parents to track behaviors, environmental triggers, interventions, and responses to interventions, so they can determine for themselves what is best for their child. The DB is fully user-modifiable. Parents should ignore any questions that they are sure are not pertinent to their child; for example, if clothing is never a problem for your child, just don't fill in that one. On the other hand, if you don't know what's triggering your child's behaviors, try filling everything out and see if patterns emerge over time. You may want to delete and/or modify any of the fields I've used, since they were designed specifically for my children. If you don't know how to do so, see the HanDBase manual, or e-mail me. I want this to be very helpful to other parents of kids on the spectrum, so please write me with feedback and/or suggestions.

This program is a comprehensive database designed to replace a traditional birth log. It makes collecting detailed information about birth events and outcomes fast and simple. It can be used to collect individual or practice statistics. Intended users are CNMs/midwives, physicians, FP and OB residents, midwifery students, and L&D staff.
Extensive pop-ups and check offs make data entry fast and simple. Fields include Name, Delivery date, EDC, G/P, Primary & Delivery provider, clinic & hospital location, FHT mode & pattern, labor progress, membrane status, pain management, anesthesia use, delivery mode, C/Section indications, NB resuscitation, perineal outcome, management of delivery complications, LOS and maternal/NB status.
This applet is more comprehensive than the ACNM Clinical Benchmarking Database; the ACNM database's purpose is to collect aggregate data about midwifery practices and to allow comparisons among practices. This applet is more detailed and is practice oriented.

Comments:
BMI (Body Mass Index) now is more widely used for assessment of weight status rather than the weight charts. The only draw back to BMI, is that it cannot take into account increased muscle mass - which is denser and heavier than adipose tissue. With aging, height should be periodically obtained because of height loss in adult life stages.
BMI chart for weights 80 lbs - 325 lbs and heights 4'7" (55 inches) - 6'8" (80 inches). Interpretation of BMI levels designated in the information area of the applet.
This applet was originally based on a previous submission but I've extended it significantly to allow you to track your RM (Repetition Maximum) values for each exercise (1-6-12 RM) as you progress. This is helpful to maintain workouts at your maximum efficency. You can also add as many additional exercises into the exercise groups as you need. There are four database files that use Relationship and Linked fields to gain the maximum value from the applet. I have loaded the exercises database with the standard Body4Life exercises plus a few extra. You can add as many as you wish including cardio exercises which are also important in the B4L program. The Program database is the entry point where you define the workout for the day (upper, lower body or cardio) and build a workout for each muscle group in the upper /lower body group. I have left some data in the RepMax database so you can see the Relationship fields in action. Once you have a new RM value you can jump through the Relationship field to the RepMax database and add new values. The third database (exercises) contains all the exercises and related information such as tempo, description etc... There is now a 4th DB used to set the current date allowing the calculation of days since last exercise within a group. This is useful in balancing your exercises over the longer term. I also have developed a paper form that works with this if you're a little squeamish about taking your PDA into the fitness areas. ENJOY!

Client Database is a set of 17 databases that interconnect to give a very useful client management database for Social workers and alcohol and drug counsellors who have large caseloads. This database is easily edited to fit just about any counselling situation where detailed reports of the clients overall living and health concerns are necessary. Reports can be easily produced using Handbase's built in report and filtering capabilities.
To use this database I have my clients listed in a client category int the resident address book that are exported to a spreadsheet and then imported into the address database within Handbase. The Client database in Handbase uses the Address database for input through a series of db popups. The other databases in the zip file are all linked to the Client database.

DietIntake was written using HanDBase version 3. It is a "relational" applet that documents food and medication intake. The "linked", "relational," "database popup" (DB popup), and "calculated" field types are used through out the application. There are ten databases. In most instances, the number of related records is displayed next to the "related" field type. Links are provided to the child databases for editing of those databases. As a note, deleting files must be done from the parent record in order to maintain proper indexing. In addition, more fields can be added to the "parent" database, DietIntake; however, with the high number of "child" databases this appelt has, additional fields in the "parent" database, DietIntake, speed will be noticeably slower due to the use of the "relational" field type.

Food and medications are maintained in this database.
"View Breakfast" allows adding/viewing of items eaten during breakfast; it uses the date as the relational factor. "Edit Breakfast" is used to edit the "DietBreakfast" database. Fields are provided to capture item, calories, carbohydrates, fat, sodium and notes. The "db popup" field type is used to bring calories, carbohydrates, fat and sodium into the record. The information brought in may be manually changed as needed.
"View MorningSnack" allows adding/viewing of items eaten during the morning snack; it uses the date as the relational factor. "Edit MorningSnack" is used to edit the "DietMorningSnack" database. Fields are provided to capture item, calories, carbohydrates, fat, sodium and notes. The "db popup" field type is used to bring calories, carbohydrates, fat and sodium into the record. The information brought in may be manually changed as needed.
"View Lunch" allows adding/viewing of items eaten during lunch; it uses the date as the relational factor. "Edit Lunch" is used to edit the "DietLunch" database. Fields are provided to capture item, calories, carbohydrates, fat, sodium and notes. The "db popup" field type is used to bring calories, carbohydrates, fat and sodium into the record. HanDBase and its application in the medical community:
Let me start off by saying HanDBase is a fantastic and very powerful relation database program for the palm pilot. Its true power comes from the fact that it is almost completely customizable and enables the user to link several databases together. Linking enables the user to jump from one database to another quickly and easily so that vital information can be accessed at a moments notice. I think this database is fantastic with only a few exceptions which I will outline later.
What I have done is taken the basic HanDBase program and created several databases to enable me to track patients in an inpatient setting. The database is set up with a "Main" database with several other databases linked to it. The "Main" database contains mostly demographic information about a patient and then linked to this database are databases which track: "VITALS", "IN/OUT", "CBC", "CHEMISTRY", "LFT", "ABG", "CARDIAC ENZYMES", "COAGS", "TO DO" LIST, "MEDS", "IVF/DRIPS", "STUDIES", AND "PROBLEM LIST".
Databases can be added or discarded as needed (very nice). I have found that it is quite easy to customize my databases for particular sub-specialty rotations that I have been on as well.
Please feel free to use these databases as you see fit. Post them, pass them to friends, etc.
Overall, this is the best thing Ive come across for patient tracking. The price is right, it is completely customizable, and it doesn't take a genius to figure out how to work it.
Anaesthesia Logbook which will allow the data to be accurately imported into the Desktop PC Royal College of Anaesthetists (UK) logbook by Hammond and McIndoe. The logbook has linked databases which makes it easier to enter data than in the "College HanDBase logbook" offered above. Version 2: fixed "operation import" error and extra fields for the college logbook V2. Also extended text file with instructions for importing data.
This logbook is intended for use with older versions of the UK College PC Logbook and HanDBase prior to version3.
N.B. For the latest version intended for use with the RCA logbook v3 together with HanDBase v3 please see HandyLogbook v3.3 entered lower down.

Comments:
Submitted as a zip file. Contains four databases and a text file. The main 'handylog' database is linked to 3 others for critical incident, procedure and operation entries.
Resubmitted Version 2 30/1/2002 to fix minor errors
Version 3 submitted separately for those wishing to use the updated version 3 logbook for the PC from the RCA UK
Revised anaesthetic logbook to accommodate the changes incorporated in HanDBase v3.0j (July 2004). The forms have also been modified to enhance appearance. Original "classic" version retained for those not wishing to use the forms; please see the HandyLog Forms Doc supplied for instructions.
Handylog has the ability to perform simple analyses while on the handheld using the new 'views' feature of HanDBase v3. The applet is designed to link with the UK PC based logbook v3 or above by Hammond and McIndoe, but can easily be used to export data into Excel or Access. It can also readily be adapted to suit personal needs. Contains 4 databases together with text files in acrobat reader and palm doc formats. The databases are linked to enable easier entry of data on the handheld, reducing the need to type with the on screen keyboard making its use as “point & click” as possible. Contains databases for operation, critical incident and procedures, otherwise it functions in the same manner as the 'RCA HanDBase Logbook'offered elsewhere. Also added are fields to automatically calculate the age of the patient
